Community Health Assessment

Paulding County Health Department strives to bring together people and organizations to improve community wellness. The community health assessment process is one way we can live out our mission. In order to fulfill this mission, we must be intentional about understanding the health issues that impact residents and work together to create a healthy community.

Community Health Improvement Plan

A primary component of creating a healthy community is assessing and prioritizing needs for impact, and then addressing those needs. In 2023, Paulding County conducted a comprehensive Community Health Assessment (CHA) to identify priority health issues and evaluate the overall current health status of the health department’s service area. Thesefindings were then used to develop an Improvement Plan (CHIP) to describe the response to the needs identified in the CHA report.
