Birth Records

Ohio records of births are filed within the registration district where the event occurred. If the birth occurred within the district, the record is on file at the Health District. If the birth occurred outside of the district, but within Ohio, the certificate can be obtained from the Ohio Department of Health.

Ways To Obtain Birth Records
You may stop in at the office and obtain a full-size certified birth certificate. We can no longer issue uncertified copies of birth certificates. Please contact the Deputy Registrar by telephone at 1-866-399-3921 or (419)399-3921 for additional information on obtaining a birth certificate.

Death Records

Ohio records of deaths are filed within the registration district where the event occurred. If the death occurred within the district, the record is on file. If the death occurred outside of the district, but within Ohio, the certificate can be obtained from the Ohio Department of Health.

The State Office has death records which occurred after December 31, 1936. Death records which occurred December 20, 1908 - December 31, 1936 can be obtained from the Ohio Historical Society Archives Library Division, 1985 Velma Avenue, Columbus, OH 43211-2497.
